  1. A Laser Hazard Assessment Report for each operable laser containing all information specified in the statement of work will be provided using EASY-HAZ™ LSO Hazard Analysis Software. A licensed copy of this software will be provided as part of the audit. The entire Class 3B and 4 laser inventory will be loaded into EASY-HAZ™ LSO Hazard Analysis Software and individual hazard analysis reports can be generated and saved as PDF files for reference. Inventory and device specification information only will be provided for lasers in storage or inoperable lasers. Multiple reports will be provided for a given laser where multiple operating conditions are identified.
  2. Review beam and non-beam control measures, Personal Protective Equipment (PPE) requirements, proper OD for laser eyewear, laser controlled areas for proper signage and entryway controls, and laser safety training requirements for employees and the LSO.
  3. Provide Problem Based Learning during the audit as necessary.
  4. Review of Laser Safety Administrative Program elements including but not limited to Laser Safety Committee (LSC) role and responsibilities, review of laser safety training materials, medical surveillance program (if applicable), and Standard Operating Procedures (SOPs). Sample Laser Safety Program and Laser Standard Operating Procedures will be provided. This proposal does not include preparation of individual SOPs, but that activity can be added as an option.
  5. A laser self-audit form with instructions for use will be provided.
  6. A final project meeting including selected faculty and staff will be performed on the last day of the audit. This meeting will address audit findings, conclusions and recommendations.
  7. A draft and final project report including recommendations for laser control measures and administrative elements will be provided.

Description

Kentek's extensive experience with large-scale audits has allowed us to refine our audit tools and techniques to make the most efficient use of our time. Our university and research facilities laser consulting services include the inspection of all laser devices, collection of all specified data, performance of required laser hazard analysis calculations, and preparation of a report for each laser including recommendations for control measures. Critical project elements are to develop a detailed plan at the beginning of the project, to reliably record all data in a useful format, and to process data and write recommendations as soon after laboratory visits as possible.

All work will be performed in accordance with the requirements ANSI Z136.1 “Safe Use of Lasers” standard, OSHA requirements for personal protective equipment (29 CFR 1910.132), and the Federal Laser Product Performance Standard (21 CFR 1040.10 and 1040.11). EASY-HAZ™ LSO Laser Hazard Analysis Software will be used to perform laser hazard analysis calculations, store all laser data and recommendations, and prepare a Laser Hazard Analysis Report for each laser.
